We teach kids how to program. Ages 5-18 come to our centers twice a week and do programming work at home the other 5 days a week.
What's your vision?
I simply need a single page (8.5"x11" american standard letter) student application. This is only to print out, nothing interactive. I've designed one in Adobe InDesign and I'm reminded how bad I am of a designer. The creativity comes from making this as simple as possible. Not a whole lot of room for making it beautiful... I also want this to be in Adobe InDesign so I can modify it as I need. Here's what I need for a "Student Application" form: 1) My company name, UCode 2) Title of "Student Application" 3) Preferred enrollment date 4) "Student Information" section with the following parts 5) Name 6) Birthdate 7) Gender (male or female) 8) Grade (PK-3, PK-2, PK-1, K, 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, other) 9) Name of School 10) Home Address 11) Phone number 12) Email 13) "Completed by Parent/Guardian" section 14) First "Parent/Guardian" section 15) Name 16) Relationship (father, mother, other) 17) Home address 18) Home phone number 19) Business phone number 20) Email 21) Another copy of "Parent/Guardian" section 22) In Case of Emergency section (name, telephone, relationship) 23) Date 24) Parent/Guardian's signature 25) Payment section 26) Type (Credit Card or Check (mention to include a voided check if choosing this option)) 27) Credit card number 28) Credit card expiration date 29) Credit card CVV code 30) Name on credit card 31) How did you learn about UCode? The numbers in the description are arbitrary. It was just a way for me to list out the requirements. My logo will be picked tomorrow from another 99designs contest, for now, just the word UCode is fine. For examples, simply do a google images search for "application form".
