Story to tell in the infographic
The FitnessDx report is a fitness "report card" that includes a persons test results -- lab values, body measurements, calculations. They will keep these report card(s) for comparisons and tracking progress over time.
Data to include in the infographic
I am looking to design "reports" (4 total: Body Composition, Basic Biomarkers, Expanded Biomarkers, Advanced Biomarkets) that each include the following.
HEADER = FitnessDx Logo, Name of Test Group (e.g. Body Composition, Basic Biomarkers, Advanced Biomarkers)
SUB-HEADER = Labeled space for a) Full Name, b) Date of Birth, c) Test Date
BODY will contain Result Blocks that each contain:
- Name of Test (abbreviation, if any)
- Description of Test
- Space for handwriting previous results (2-3 + date in MO/YR format)
- Graph with risk categories
- Result space (for hand written) -- also include "units" of measure in "Result" label
Elements to include in the infographic
On 8.5 x 11 inch paper/card stock. Light colored background preferred for printing and marking ease.
I want the reports to match with my overall design and feel of the logo I uploaded -- clean and modern. My medical practice branding is similar -- website at http://neu.care, but I'd like more color (such as the points in logo) and high-tech feel to this project. The lab testing service, http://WellnessFX.com, is very similar to the design look and feel I'd like to achieve but in a print format.
I attached my vision of a "result block" here, but feel free to tinker with the layout of text and data. I think a vertical "bar graph" for risk category would be best and I would like each block to have a similar layout.
I have attached my logo here, as well as several documents with text outline to be included in reports. I also did a basic sketch of a "layout" of a result block (For Hemoglobin A1c) that I envision.
