I need a double sided Fold-Over Business Card designed for Premier Eye Associates. Please submit your design as a straight on shot (not angled) on a plain black background. Please do not include my name or any name in the design. I also included some old versions to show you an idea of what types of designs I like. Below is what should be include on the front of the card.
